Coming Soon: Updated Plan Establishment Kits

Effective June 15, 2026, the Plan Establishment Kits (PEK) will be updated to reflect changes pertaining to the IRS interim amendment for SECURE 2.0. Beginning August 1, 2026, prior versions will not be accepted.

- Participant Notice of Blackout
  
  This notice must be provided to participants if certain transactions are suspended during a transition to a new provider.
- Sample Board Resolution
  
  C corporations, S corporations, and LLCs can use this form to document their plan-related decisions.
- ERISA Section 404(c) Employee Information Sheet
  
  This form provides the required information to participants for the employer to comply with ERISA Sec. 404(c).
- Designation of Beneficiary Form
  
  Participants use this form to name beneficiaries for their accounts.

- 401(k) Safe Harbor Contribution Notice
