These forms are provided for qualified retirement plan administrators to use in the course of administering their plans. The plan administrator should keep copies of these completed forms.

- Sample Board Resolution
  
  C corporations, S corporations, and LLCs can use this form to document their plan-related decisions.
- Participant Notice of Blackout
  
  This notice must be provided to participants if certain transactions are suspended during a transition to a new provider.
- ERISA Section 404(c) Employee Information Sheet
  
  This form provides the required information to participants for the employer to comply with ERISA Sec. 404(c).
- ERISA 404(c) Employer Guidelines
  
  This guide helps employers understand and comply with the requirements of ERISA Sec. 404(c).
- Designation of Beneficiary Form
  
  Participants use this form to name beneficiaries for their accounts.
- Document Service Agreement (DSA)
  
  Document Service Agreement outlines Ascensus’ and the employers' responsibilities.
- Owner-Only (Individual 401(k)) Plan Design Questionnaire
  
  A 401(k) plan designed for sole proprietors with no employees, or for owners and their spouses, offering elective deferrals and/or profit sharing contributions.
- 401(k) Profit Sharing & Money Purchase Plan Design Questionnaire
  
  A 401(k) Profit Sharing plan is designed for businesses with employees, offering elective deferrals, profit sharing contributions, and employer matching contributions. (Elective deferrals and employer match are not allowed in a profit sharing only plan.) A Money Purchase plan requires a fixed annual employer contribution. (Elective deferrals and other discretionary employer contributions are not allowed in a money purchase pension plan).
- SIMPLE 401(k) Notice
  
  Employers with a SIMPLE 401(k) plan must distribute this notice annually to plan participants.
- Automatic Rollover Notice
  
  Employers use this form to notify plan participants of automatic rollover provisions applicable to their plan.
- Successor Trustee for Individual (k) plans
  
  Plan Sponsors use this form to designate a Successor Trustee
- 401(k) Safe Harbor Contribution Notice
  
  Employers with a 401(k) safe harbor plan must distribute this notice annually to plan participants.
- QRP Update Request Form
  
  Plan Sponsors use this form to request changes to their contact information and/or to update provisions within their existing Adoption Agreement serviced at Ascensus.
