The Plan Establishment Kits (PEK) have been updated to reflect changes related to the IRS interim amendment for SECURE 2.0. Please begin using the updated materials immediately. Beginning August 1, 2026, older versions will no longer be accepted.

- Participant Notice of Blackout
  
  This notice must be provided to participants if certain transactions are suspended during a transition to a new provider.
- ERISA Section 404(c) Employee Information Sheet
  
  This form provides the required information to participants for the employer to comply with ERISA Sec. 404(c).
- ERISA 404(c) Employer Guidelines
  
  This guide helps employers understand and comply with the requirements of ERISA Sec. 404(c).
- Designation of Beneficiary Form
  
  Participants use this form to name beneficiaries for their accounts.
- Successor Trustee for Individual (k) plans
  
  Plan Sponsors use this form to designate a Successor Trustee
- 401(k) Safe Harbor Contribution Notice
  
  Employers with a 401(k) safe harbor plan must distribute this notice annually to plan participants.
- QRP Update Request Form
  
  Plan Sponsors use this form to request changes to their contact information and/or to update provisions within their existing Adoption Agreement serviced at Ascensus.
- QRP Distribution Notice
  
  This form contains all of the IRS-Required information (under IRC Sec. 402(f)) that a plan administrator needs to provide to participants when distributions are made.
- QRP Application for Distribution
  
  This form is used by particiapnts to request distributions and waive the qualified joint and survivor annuity and federal income withholding.
